Who needs to be present for my child’s appointment?
A parent/legal guardian must be present for your child’s 1st appointment and for any treatment plan consultations.
For routine appointments, your child can be accompanied by an adult who has been authorized by a parent/legal guardian. If you have not provided our office with this information, please complete ourConsent to Treat Minor Patient form
Can I bring my other children along to my child’s appointment?
We ask that any family with three or more children have two adults at the appointment.
Do you require masks to be worn in the office?
We strongly encourage all patients and their guests to wear a face-covering while in our office.
What should I do if my child is sick before their appointment?
If any member of the family is currently experiencing any of the following: cold, fever, congestion, stomach bug, etc please contact our office prior to your appointment.
We are thrilled to welcome the staff from Lively Hive to Tooth Fairy Time. Join us at CVPD on May 16 from 5:30-7pm to play with us!
The staff from Lively Hive will set up engaging & crowd-favorite games and activities through stations that will focus on play, sensory, and motor skills at Tooth Fairy Time. The planned games and activities are geared mostly for ages 0-6, but all ages are always welcome, and the older kids can certainly help the younger friends/family members. Reserve your spot, today!
